North Derbyshire care system to help the frail

Health leaders have joined forces with the voluntary sector to launch a service to help frail and elderly people stay at home rather than go to hospital.

NHS North Derbyshire, Hardwick and Erewash Clinical Commissioning Groups have contracted North Derbyshire Voluntary Action to provide a voluntary sector single point of access service which goes live on Monday.

The service links people at risk of hospital admissions to support services. Patients will be referred to vSPA by care professionals. Voluntary organisations can refer their clients to the vSPA service for support. The service will be based at Clay Cross Hospital. The vSPA will be supported by a worker who will find ways of helping patients to live independently.
